So... What is a Care Plan?
A home care plan is simply a document that is created by the client and a health care professional (either our Care Team or a social worker) to meet the needs of the client's care.
Everyone client will demand different care for themselves and with the help of healthcare professionals, we are able to create a document which contains all their needs which put the client in control.
The Great Care Company ensure that all our clients have a completed care plan in their homes. They are used to so carers can read upon what they do at each client’s and to also have access to their next of kin, social worker and doctor in case of an emergency.
What Should A Care Plan Include?
Care Plans should be detailed and include the following details:
Your or your loved one's eligible needs.
The goals you have agreed you would like in the plan and how to achieve them.
The services you'll receive to help you achieve your goals, and who is responsible for providing these.
Whether you will receive these services directly from your local authority, or if you will receive direct payments or a personal budget so that you can choose your own services.
Any differences in opinion that you, or the person you care for, have had with the local authority about the assessment or care planning process
The name of the person responsible for implementing, monitoring and reviewing the care plan.
The goals you have agreed you would like in the plan and how to achieve them.
A risk assessment, including any actions that will be taken to manage and reduce risks.
A plan for dealing with emergencies.
The cost and charges for the services.
The date of the next review of the care plan.
Any support that you or other carers are willing and able to provide, and your views.
Progress notes to keep up to date with documented information of each call.
Forms for medication, food, finance, body maps, complaints etc…

Care Plans should always be kept up to date and all forms that need to be documented or signed must be completed by whoever is responsible.
