Reablement Care
If you need assistance at home while recovering from an accident, illness, or hospital stay in Lincolnshire or Yorkshire, The Great Care Company offers top-notch care to meet your needs. Our carers provide support for your physical and emotional well-being, tailored to your requirements and arranged promptly for as long as necessary.
What Is Reablement Care?
Reablement care (often known as intermediate care, post-surgical or post-operative care) is a home care service you can receive after an illness, operation, accident or hospital stay. It’s usually only required for a short period of time, typically a few weeks, whilst you adapt to your new situation or recover enough to look after yourself.
What ages do we care for?
The Great Care Company works with people over the age of 18. As a rule, we mostly look after people who are 50 years plus, or over retirement age, as this is often the age range that will require the type of services we offer.
How long is Reablement Home Care required for?
This is usually a short-term service that may only be needed for a couple of weeks. On average, the service is required for about 6 weeks but this will vary, case by case. We will understand your unique circumstances once we’ve spoken to you.
Who is it for?
This service is ideally aimed at people who will be making a recovery after spending some time in the hospital. It could be due to an illness, an operation or an accident. Our carers are there to make sure everything is running smoothly in terms of your ongoing treatments, medication, therapies or tasks around your home until you are fully able to regain your independence.
What are the benefits of Reablement Home Care?
Intermediate Home Care works because it looks at your recovery goals and sets milestones to work towards. Seeing notable physical recovery really benefits the mental outlook of the person concerned and helps them feel like there is a goal and an end date to feeling fully independent again.
What about hours and times?
The great thing about our home care services is that they are completely customisable to suit your exact needs and budget. Depending on your unique circumstances, you may require a few visits per week, per day or complete live-in care. We’re here to assist with everything you need, without any hidden costs, as the times you require us there.
Will this affect my loved ones?
Our carers are trained to provide the very best levels of care whilst respecting the privacy and dignity of the people they are supporting. They are matched to you and will be there to provide not only the physical support and services required but also emotional support to those in their care, their family, their friends and even their pets!
Home Carers To Help You Regain Independence.
Upon hospital discharge (depending on your age and circumstances) you may have been offered support in the form of social care or access to a care home.
This is often not the best solution.
Private reablement home care offers a unique solution, where you are encouraged to do things for yourself with assistance along the way. Our team want to see you gain your independence until you’re in a comfortable place to lead the life you had before the hospital visit.
The Great Care Company keeps families together whilst giving your loved ones some help if they have assumed the role of a carer whilst you recover.
We’re here to take the burden away from recovery. Both for the persons in our care and their families who may need a break from care or need to be able to undertake tasks in their own lives such as work, looking after other family members or simply needing some respite.
From setting recovery goals and aiding with your physical and emotional needs to helping with tasks around the home, our carers will support you and your loved ones every step of the way.
We work to promote your independence and regain your abilities until you are fully able to undertake the tasks that you may have taken for granted before your time in hospital.
Our services include the setting of goals, help with medication and physical therapy as well as basic tasks like hygiene assistance, cooking, cleaning and even shopping.
We offer care in the form of encouragement. We want to see you achieve the things that make you feel better mentally, emotionally and physically. Our job is done when you have your independence back and no longer require support in the form of care. Instead of simply doing the tasks for you, we take a measured approach on working with you until you can do them for yourself.
What Services Does Elderly Home Care Provide?
Assistance with Personal Hygiene
It may be difficult to clean yourself or you may not want your relatives to assist with this. We understand that. Our carers respect your dignity and privacy whilst ensuring you are bathed or showered and will encourage you to undertake the task as much as your are able to.
Setting Recovery Goals & Milestones
It’s vital that you have recovery targets in place to aid you in regaining your independence. We’ll work with your health care advisors to make sure a realistic, safe and obtainable plan is in place whilst we assist you to regain the skills you need to live the life you want.
Physical Therapy & Medication
Our team will ensure you’re given the correct medication at the pre-set times and help with therapies and routines. Physical therapy, exercise and practising skills will be conducted as per the recommendations of your doctors, surgeons or consultants. We will make sure nothing is missed out and you feel empowered to keep going until you feel comfortable doing things yourself.
Shopping, Meal Planning & Cooking
When people are recovering, it can be difficult to keep track of things like shopping, cooking and eating. Our carers will help to make sure all of this is taken care of and you’re getting a nutritious diet that is beneficial to recovery whilst staying properly hydrated. We can help with everything from buying food, cooking and making drinks to the washing up afterwards.
Home Visits Or Around The Clock Care
We work hard to understand your unique circumstances and requirements. You may require short term help or sustained support over a period of time. We’re here for you, whether it may need a live-in carer to be with you 24/7 or simply a few visits a week to take care of basic tasks. We’ll assist in the ways you want us to until you feel able to take over.
Laundry & Housekeeping Services
The road to recovery can often take priority over basic things like cleaning and tidying. Rest assured, if you or your loved ones are finding it a challenge to balance care with daily tasks then we can help. What may be a physically demanding task to you right now, is something we’re equipped to help with.
Keeping Your Social Life
Having a social life, seeing friends, keeping hobbies and doing things we enjoy is a key part of being happy and a great way of finding recovery. Our carers work around you to ensure you don’t miss out on anything. We’ll even aid you to do things that may be difficult at the moment and keep appointments and schedules on your behalf, accompanying you as required.
Physcial & Emotional Support
We pair our carers with our clients in a unique way and match them in terms of care as well as personality. They are here to provide an extension of your family and support group, giving companionship and encouragement when you need it the most.
